With this service the patient can inform you of a change of address, or other important details, such as phine numbers.

How the service works

• Patients compelete a change of address (or other details) form online.
• The form is tailored to your requirements and can include any additional information you require.
• The request can be processed at any time by the practice, when it is convenient.
• When processed by the practice a reassuring confirmation response will be automatically sent to the patient.
• Fully secure - no part of the request or response is sent by email.
• If a patient consistently uses this service inappropriately, or is abusive, the Nurse can block that patient from sending further queries.
